Cello Advantage Health, an independent consulting firm, will be conducting a survey to better understand the ongoing needs of the FOP community.

Participants will be asked to complete a one-time, online survey, which is estimated to take 30 to 45 minutes to complete. 

We are looking for participants who have a confirmed diagnosis of FOP.

• If necessary, a caregiver or aid can help you fill out the survey.
• If you are currently participating in a clinical trial, you can still participate in this survey.
• If you are under 18 years of age, please have your parent/caregiver participate in filling out the survey with you.
• The survey is written in English, therefore, understanding of English is required.

Participation is completely voluntary. If you are interested, please send an email to [email protected]. If you're the parent/caregiver responding on behalf of someone with FOP, please include their name in your email. Participation is limited, so you'll want to volunteer early.

Please be assured that the information collected will be kept in strict professional confidence and findings will be reported in the aggregate.
