FOPonline Enrollment Form

Participation in the FOPonline newsgroup is open to IFOPA members who have paid their annual dues, as well as people afflicted with FOP, their families, medical professionals with an affiliation to someone with FOP and people with personal connections to someone with FOP.

If you are an IFOPA member afflicted with FOP, or an immediate relative, and you wish to request a waiver of the IFOPA membership dues because of financial difficulties, please check the appropriate box below and a member of our staff will contact you via email.
