FOP Symposium Family Workshop Sign-up Form
JW Marriott Orlando, Grande Lakes , Florida This is the electronic version of the official sign up sheet for the workshops that will be held during the symposium. It is important that each symposium attendee sign up for the workshops they will be participating in. Please check the appropriate box of the workshops each symposium attendee plans to attend. Note: The form needs to be filled out once for each person wishing to attend one or more of the workshops. To add additional people, please submit this form and then return to the page, click "RESET" at the bottom of the page and fill out the form again with the information of the next person. Workshop #1 is a two part series that is designed to be informative to specific groups. Please sign up only for the group that applies to you.
